HEALTH/WELFARE BENEFITS SUMMARY

(pre-tax premiums via cafeteria plan)

Unitron’s medical insurance utilizes United Healthcare Cost Plus PPO as the national preferred provider network.  New employees become eligible for coverage the first of the month following (60) sixty days of employment.  

  • In-network annual deductibles are $800 individual/$2,400 family 
  • Annual out of pocket costs for in-network services are $6,000 per person/$12,000 per family 
  • Most preventative services are covered at 100% after copay
  • Most diagnostic services are covered at 80% after deductible  

Please refer to the summary plan document for specifics as well as limitations and exclusions.  Out of network services are covered but are subject higher deductibles, out of pockets and coinsurance levels.  

RX coverage through Verus-RX with tiered copays of $5 generic /$30 formulary /$65 brand name for pharmacy purchases.  New employees become eligible for coverage the first of the month following (60) sixty days of employment. Mail order options are available at a further discount for maintenance medications and ScriptSourcing offers $0 copays for International Pharmacy services. 

A monthly premium subsidy to all full-time employees equaling $375 ($4,500 annually) is given to offset
premiums for welfare benefits elected by the employee (whether Unitron benefits or not).

Unitron offers dental insurance plans through Equitable.  New employees become eligible the first of the month following sixty days of employment.

  • Dental PPO/High plan covers preventative services at 100% and allows employees to choose either in-network or out-of-network providers
  • Dental PPO/Low plan provides similar coverage for routine in-network services at a lower cost, but excludes coverage for certain major services such as orthodontia, dentures and root canals

Unitron offers vision insurance through Equitable/VSP.  New employees become eligible for coverage the first of the month following (60) sixty days of employment.

  • Vision Plan offers annual eye exams for a $10 copayment when using participating providers
  • Corrective lenses are available at discounted rates with fixed copayments

  • Unitron provides group term life insurance/AD&D through Equitable equal to two times annual earnings, at no cost to you.  New employees are automatically enrolled the first of the month following sixty days of employment. 
  • Supplemental term life/AD&D coverage for you, your spouse and your children are available if you choose to purchase it.   Late enrollment in supplemental coverage or coverage amounts over the guaranteed issue requires evidence of insurability.
  • Unitron provides disability coverages through Equitable.  Eligible employees are added to these plans on the first of the month following sixty days of employment.  
  • Short Term Disability (STD) coverage (equal to 60% of your weekly wage up to $600) is provided at no cost to all full-time employees per plan details.
  • Long Term Disability (LTD) coverage (equal to 60% of your monthly wage up to $8,000) is provided at no cost to all full-time salaried employees per plan details.
  • Long Term Disability (LTD) is also made available to hourly employees on an elective basis at favorable group rates per plan details.

PAID TIME OFF

Paid vacation time is made available to full-time employees after the completion of one year of continuous employment.  Vacation pay counts towards overtime for Hourly employees.  Full-time employees with one or more years of service are eligible for paid vacation based on the following schedule:

  • 1-10 years = 10 days
  • 10-20 years = 15 days
  • 20+ years = 20 days

All regular full-time employees are eligible for paid holidays upon hire.  Holiday time counts towards overtime for Hourly employees.  The following holidays are observed each year:

  • New Year’s Day
  • Memorial Day
  • 4th of July
  • Labor Day
  • Thanksgiving Day
  • Friday after Thanksgiving
  • Christmas Day

All full-time Unitron employees are eligible for Personal Time-Off (PTO) of 40 hours per year to be used for personal business or illness.

  • PTO is given on January 1 of each year or pro-rated for new employees
  • PTO does not accumulate from one year to the next - unused PTO hours are paid out to hourly employees at the employee’s regular rate the first payroll of the following year

All employees will be granted paid time off to arrange for or attend the funeral of an immediate family member.

  • Immediate family members include  spouse, child, parent, father/mother-in-law, grandparent, sibling, and grandchild
  • A maximum of (3) three days of regularly scheduled days will be granted

Unitron encourages employees to fulfill their civic responsibilities by serving jury duty or witness duty.  Employees will receive full pay for up to (5) five regularly scheduled days off per occurrence.

RETIREMENT BENEFITS

Unitron offers a 401(k) Retirement Savings Plan through Fidelity.  

  • The plan permits both pre-tax and Roth contributions
  • Match formula is 50% of elective employee contributions up to 6%; effectively providing employees who contribute 6% of pay or more with a 3% employer contribution  
  • Employer match is subject to a stepped 6-year vesting schedule
  • Employees are automatically enrolled at 3% level unless they choose otherwise  
  • Employees may defer up to 100% of earnings into the plan  
  • Employees can roll other plan monies into the plan immediately upon hire and become eligible to participate the first of the month after completing 3 months of service 
  • An automatic increase program is available and will bump up the deferral rate by 1% per year until it hits 6%.  Automatic increases are not applied to deferral rates of 6% or higher.
